New Leaf Peer Support LLC is building supportive independent living pathways for youth aging out of foster care and adults who need structure, mentorship, and peer support while transitioning toward independence and long-term housing stability.

We recognize that aging out of the system or navigating major life transitions can leave individuals without consistent support, stable housing options, or guidance during one of the most critical periods of their lives. Our housing model is designed to help close that gap by providing safe housing combined with peer support and stabilization services.

Transitional Housing (Shared Housing)

Our transitional housing program provides a structured shared living environment for individuals who need stability and support while working toward permanent housing.

Pricing Based on Income No INCOME NO PROBLEM 

Participants receive peer support and housing stabilization services while developing the skills and routines necessary for long-term independence.

Bridge / Stabilization Housing (Short-Term Placement)

Our Bridge Housing program provides short-term placements for individuals who need immediate stabilization while transitioning to permanent housing.

Bridge placements are often used for individuals who are:

Awaiting permanent housing placement

Transitioning from treatment programs or detox

Being discharged from hospitals or healthcare facilities

Navigating reentry after incarceration

Experiencing housing crises that require short-term stabilization

Services Provided

New Leaf Peer Support offers non-clinical services designed to support housing stability and long-term independence.

Our services include:

Peer support and mentorship

Housing navigation and placement assistance

Case management and stabilization support

Connection to community resources and benefits

Transportation assistance when needed

 

Participants who demonstrate responsibility, engagement, and progress within the program may have opportunities after 9–12 months to transition into more independent housing options, including potential placement within a supportive tiny home or mobile home community designed to help residents move toward full independence.

Our approach to independent living is peer-led, non-clinical, and accountability-focused. Housing is not the solution on its own — support, mentorship, and life skills are what create long-term success.

Young adults are supported while they learn to manage daily responsibilities and build sustainable routines.

Role of Peer Support

Peer Support Coaches play a central role in our independent living pathway. Peers walk alongside participants, offering encouragement, accountability, and guidance based on lived experience.

Peer support may include:

Regular check-ins and goal tracking

Support with routines and time management

Life skills coaching and problem-solving

Encouragement through challenges and setbacks

Support navigating systems and resources

Peer support is non-clinical and focused on empowerment, not control.

Housing & Facility Development

New Leaf Peer Support LLC is actively developing facility-based and partnership housing options to support independent living for youth aging out of foster care.

Housing options will be offered through:

Licensed facilities

Community partnerships

Transitional and supportive living arrangements

Availability of housing is dependent on licensing, partnerships, and program readiness. Peer support services remain available regardless of housing placement

Clinical treatment or therapy

A locked or restrictive environment

A permanent housing solution/Referrals are available New Leaf will soon be offering Tiny Homes to program participants rent and rent to own options will be available  

A replacement for personal responsibility

Our goal is to help  adults move forward — not create dependency.
