Who We Are
New Leaf Peer Support is a structured mental health and support program focused on helping individuals stabilize, heal, and transition into independent living.
We combine licensed therapy services with peer-led support to provide a balanced approach to care—addressing both clinical needs and real-life challenges.
Our team works alongside each individual to build structure, accountability, and practical life skills that support long-term success beyond treatment.
Who We Serve
We support adults who are navigating life transitions and need structured support to regain stability and independence, including:
- Individuals experiencing housing instability or homelessness
- Adults reentering the community after incarceration
- Individuals discharging from treatment, hospitals, or crisis care
- Adults facing mental health challenges who need consistent structure and support
- Young adults aging out of foster care or lacking stable support systems
Our program is designed for individuals who are ready to engage in services and benefit from a combination of therapy, peer support, and guided independent living support.
Youth Aging Out of Foster Care (18–24)
Young adults transitioning out of foster care who need structure, guidance, and consistent support to build independence.
Young Adults Facing Housing Instability
Individuals navigating homelessness, unstable housing, or transitional living while working toward long-term stability.
Adults in Life Transitions 18+
Youth and young adults impacted by reentry, system involvement, or major life changes who benefit from peer mentorship and accountability.
