New Leaf Peer Support LLC provides peer-led housing stabilization and support services for youth aging out of foster care and adults navigating homelessness, reentry, and life transitions. Our programs combine supportive housing, peer mentorship, and community resource coordination to help individuals achieve stability and transition into permanent housing.

WHO WE SERVE
Youth Aging Out of Foster Care (18–24)
Young adults transitioning out of foster care who need structure, guidance, and consistent support to build independence.
Young Adults Facing Housing Instability
Individuals navigating homelessness, unstable housing, or transitional living while working toward long-term stability.
Adults in Life Transitions 18+
Youth and young adults impacted by reentry, system involvement, or major life changes who benefit from peer mentorship and accountability.

Peer Support Coaching
One-on-one peer support from trained specialists with lived experience. We focus on goal-setting, accountability, and real-life problem solving.
Life Skills & Independent Living Support
Support with routines, budgeting, appointments, employment readiness, and daily living skills needed for independence.


Supportive Living Pathway
A step-by-step pathway toward supportive and independent living as we transition into facility-based housing options for eligible young adults.
